Founder & Product Compliance Lead Review

  • Recommendations received from the Clinical Director

Decision: Close at this stage

  • Determines whether policy changes, compliance actions, technical controls, external consultation, or additional risk mitigation are required
  • Approves and documents final actions, assigns responsibilities, ensures implementation and follow-up
  • Where appropriate, initiates formal incident management, governance review, or regulatory review

Documentation requirements

For all escalated matters, the following must be recorded and retained:

  • Date and time of escalation
  • Role responsible for review at each stage
  • Decisions made at each stage
  • Audit trail of actions taken
  • Closure status and review outcomes
  • Records retained per platform governance and compliance requirements
