Why CPTSD needs a slightly different approach

Complex PTSD usually develops from repeated experiences over time rather than a single event, and it often brings emotional flashbacks — sudden floods of old feeling without clear images or memories. Because the trigger may not be obvious, grounding is less about identifying what happened and more about steadying the state you are already in.

Attention turned inward can also intensify things when the inside is where the distress lives. Starting with the outside — the room, the floor, an object — is often gentler.

Exercises that tend to suit complex trauma

  • Orienting: slowly turn your head and name six things in the room.
  • 5-4-3-2-1, done out loud, working through sight, touch, sound, smell and taste.
  • Temperature: cold water on the wrists, or holding something warm.
  • Feet and floor: press down, notice the surface, describe it in detail.
  • Longer out-breath, eyes open, for six to ten breaths.
  • Containment: hands crossed over the opposite shoulders, with a little pressure.

What to be careful with

  • Long closed-eye practices, especially early on.
  • Deep breathing that becomes fast or forced.
  • Extended body scans when numbness or dissociation is already present.
  • Treating a difficult attempt as failure — a technique not suiting you is information, not a setback.

Building a routine you will actually use

Choose two exercises that felt tolerable on a calm day and practise them when nothing is wrong. Practised early, they become familiar enough to reach for when things are harder — which is the whole point. Two minutes, twice a day, tends to do more than an occasional long session.
